Our adult family home combines individualized attention with skilled nursing support for adults who need ongoing respiratory care, chronic-disease management, tube-feeding support, and other complex medical services.
24-Hour Specialized Nursing Care
Residents receive around-the-clock licensed nursing support from professionals experienced in complex medical and respiratory care. Our team closely monitors each resident’s condition, medications, treatments, comfort, and individualized plan of care.
Ventilator & Tracheostomy Care
Sacred Life provides long-term ventilator management and tracheostomy care in a residential setting. Our trained nursing team provides routine trach care, suctioning, airway and oxygen monitoring, secretion management, prescribed respiratory treatments, and coordination with physicians and respiratory therapists.
Pulmonary & Airway Management
Our nursing team follows each resident’s prescribed pulmonary plan, including respiratory assessment, oxygen monitoring, suctioning, secretion management, and ordered oxygen, nebulizer, or airway-clearance treatments. Changes in respiratory status are promptly communicated to the appropriate provider or respiratory therapist.
Heart & Chronic Disease Support
Our nursing team supports residents managing heart disease and other chronic conditions by following physician orders, administering prescribed medications, monitoring for changes, and coordinating concerns with the resident’s healthcare providers.
Diabetes Management
Licensed nurses follow each resident’s prescribed diabetes plan, including blood glucose monitoring, medication administration, nutrition support, and observation for changes that may need provider follow-up.
Feeding Tube Care & Nutrition
We provide prescribed G-tube, J-tube, and PEG-tube care and feedings. Nurses monitor the tube site, feeding tolerance, hydration, and comfort while coordinating nutrition concerns with the resident’s healthcare team.
IV Care & Treatment Support
Our licensed nursing team provides IV care and management as ordered, monitors the access site and the resident’s response to treatment, and communicates concerns promptly to the prescribing provider.
Catheter Care
Residents who require urinary catheter support receive routine catheter care, monitoring for comfort and output, and observation for signs of complications. Changes are documented and communicated to the appropriate provider.
Wound & Skin Care
Our nurses provide prescribed wound care, skin assessments, dressing changes, and prevention measures. We track healing and coordinate changes or concerns with physicians and wound-care specialists.
Medication Management
Licensed nurses administer medications according to each resident’s orders, maintain accurate schedules and records, monitor for side effects or changes, and coordinate medication questions and refills with providers and pharmacies.
Antibiotics & Specialized Treatments
Our team carries out prescribed antibiotic therapy and other specialized medical treatments, monitors each resident’s response, and coordinates closely with physicians, pharmacies, and other clinicians.
Appointments & Transportation Coordination
We help schedule medical appointments and coordinate transportation so residents can stay connected with their physicians and specialists. Our team also shares relevant updates with providers and keeps families informed.
Individualized Plan of Care
Each resident has a personalized plan of care reviewed by our head RN and the residence physician. The plan reflects the resident’s medical needs, treatments, preferences, and care goals, and it is updated as needs change.
We coordinate with families, physicians, respiratory therapists, pharmacies, and other clinicians so everyone understands the resident’s current plan and progress.
