Privacy Policy

Sacred Life LLC
Effective date: October 19, 2026

Thank you for visiting Sacred Life. We know that reaching out about care for someone you love, or for a patient you’re helping, is personal. This policy explains, in plain language, what information we collect, why we collect it, and how we protect it.

Quick summary

  • We collect only what we need to answer your question and talk with you about care, admissions or referrals.
  • We do not sell your information, and we don’t use it for advertising.
  • We share it only with a few service providers that help us run our business (like our email provider), or when the law requires it.
  • You can ask us to show you, correct or delete your information at any time.
  • Washington residents have extra rights over “consumer health data.” See the Consumer Health Data Privacy Policy below.

1. Who we are

Sacred Life LLC (“Sacred Life,” “we,” “us”) is a licensed adult family home in Vancouver, Washington. We provide 24-hour nursing care for adults who need tracheostomy and ventilator support.

  • Address: 11412 NE 50th Ave, Vancouver, WA 98686
  • Phone: 360-721-4303
  • Email: sacredlifehome@gmail.com
  • Website: https://sacredlifecare.com

This policy covers our website, our Facebook and Instagram lead forms, and the inquiries you send us by email, phone or message. It does not cover the medical records of our residents. See Section 11.

2. What we collect

Information you give us

  • Facebook/Instagram lead forms (Meta instant forms): your name, phone number, email address, the city or area you’re in, and your answers to a few multiple-choice questions, such as whether you’re a family member, friend or healthcare professional, what you’d like (a tour, a call, referral information) and your timeline. Our forms don’t ask about anyone’s medical condition.
  • Our website contact form: your name, email address and the message you write.
  • Email, phone calls, texts and Facebook Messenger: whatever you choose to share with us, such as your contact details and questions about care, availability or admission. Please share only what you’re comfortable with. We’ll ask for detailed health information only later, if you decide to move forward with an admission conversation.

Information collected automatically

  • Basic technical information. Like most websites, our web server and hosting provider may automatically log technical details such as your IP address, browser type, the pages you visit and the time of your visit. This keeps the site secure and working.
  • Cookies. Our website is built on WordPress. It uses only the basic cookies needed for the site to work (for example, for site administrators who log in, or to remember your details if you leave a comment on a blog post). We don’t currently use advertising cookies, the Meta Pixel or website analytics tracking. If that changes, we’ll update this policy first, and where the law requires it, we’ll ask for your consent before using them.
  • Comments. If you comment on a blog post, WordPress collects the name, email and comment you enter, plus your IP address and browser information to help detect spam.

3. How we use your information

We use your information only to:

  • Respond to your inquiry and follow up by phone, text or email, as you asked
  • Talk with you about our care, availability, tours, admissions or professional referrals
  • Keep simple records of who contacted us and how they heard about us
  • Keep our website and communications secure
  • Meet our legal and licensing obligations

We don’t use your information for unrelated marketing, and we don’t sell it or rent it to anyone.

4. When we share information

We share information only in these limited cases:

  • Service providers that help us operate. For example: our email provider (Google/Gmail), our website hosting provider, and Meta’s lead tools that deliver your form to us. Where possible, they may use your information only to provide services to us.
  • People you ask us to coordinate with, such as a hospital case manager or family member, but only with your permission.
  • Legal reasons: when required by law, a court order or a government agency (for example, licensing or adult-protection authorities), or to protect someone’s safety.
  • Business changes: if Sacred Life is ever sold or reorganized, information may transfer to the new owner under this policy’s protections.

We do not sell personal information or consumer health data.

5. Meta (Facebook and Instagram)

When you fill out one of our lead forms on Facebook or Instagram, Meta Platforms, Inc. runs the form and sends your answers to us. Meta collects and uses information under its own privacy policy, which we don’t control: https://www.facebook.com/privacy/policy/. You can manage your Facebook and Instagram ad preferences in your Meta account settings.

6. How long we keep information

We keep inquiry information only as long as we need it to respond and follow up, and then for a reasonable period for our records: generally no longer than 2 years after our last contact if no admission takes place. After that, we delete it unless the law requires us to keep it longer. If a person becomes a resident, their records are kept as required by Washington adult family home rules and healthcare laws. You can ask us to delete your inquiry information sooner (see Section 8).

7. How we protect information

We use reasonable safeguards: password-protected accounts with two-step verification where available, limited access for staff who need the information, and secure, reputable service providers. No method of sending or storing information is 100% secure. Please don’t send detailed medical records through our website form or Facebook. Call us instead and we’ll arrange a secure way to share them.

8. Your choices and rights

You can contact us at any time to:

  • Ask what information we have about you and get a copy
  • Correct information that’s wrong
  • Ask us to delete your information
  • Tell us to stop contacting you (just reply “stop” to a text, ask us on the phone, or email us)

How to make a request: email sacredlifehome@gmail.com with the subject “Privacy Request,” call 360-721-4303, or write to us at 11412 NE 50th Ave, Vancouver, WA 98686. To protect your privacy, we may ask a few questions to confirm your identity, such as the phone number or email you used to contact us. We won’t charge you, and we won’t treat you differently for making a request. Washington residents have additional rights, described below.

9. Children

Our services are for adults, and our website and forms aren’t directed to children under 13 (or under 18 for our lead forms). We don’t knowingly collect information from children. If you think a child has sent us information, contact us and we’ll delete it.

10. Links to other sites

Our website and social media pages may link to other websites, such as Facebook, Google Maps or resources for families. We’re not responsible for their privacy practices, so please read their policies.

11. Residents’ health information (HIPAA)

This policy covers inquiries and website visitors. Health information about our residents is handled separately, under the laws that apply to resident records. Where it applies, the privacy of residents’ protected health information is described in our separate Notice of Privacy Practices, available by request. Information that is protected health information under HIPAA is governed by that notice, not by this policy.

12. Changes to this policy

We may update this policy from time to time. When we do, we’ll change the effective date at the top. If we make a significant change, such as starting to use new tracking tools, we’ll post a clear notice on our website and, where required, ask for your consent first.

Consumer Health Data Privacy Policy (Washington My Health My Data Act)

Effective date: October 19, 2026

Washington’s My Health My Data Act (RCW 19.373) protects “consumer health data”: personal information that identifies, or could reasonably be linked to, your past, present or future physical or mental health. This section explains how Sacred Life handles that kind of information and how you can use your rights.

Categories of consumer health data we collect, and why

Because Sacred Life provides specialized nursing care, the simple fact that you contacted us could suggest that you or someone close to you may need tracheostomy or ventilator care. We may collect:

  • Inquiry information that may suggest a health need: your contact details together with the fact that you asked about our care, tours or admissions, and your answers about your role (family member, friend or healthcare professional), what you want and your timeline
  • Health details you choose to share by phone, email, message or our contact form, such as general information about a loved one’s care needs, equipment or upcoming hospital discharge
  • Technical information (such as an IP address in our server logs) connected to visits to our website pages about our care services

Purposes: we use this data only to respond to your inquiry, discuss whether our home may be able to meet a person’s care needs, schedule tours or calls, coordinate referrals you ask us to coordinate, keep basic records, keep our systems secure and meet legal obligations. We collect it only as needed to provide the information or service you asked for, or with your consent. We won’t collect new categories of consumer health data, or use it for new purposes, without telling you first and getting your consent.

Sources

We get consumer health data from:

  • You directly (forms, email, phone, text, Messenger)
  • Meta (Facebook/Instagram), which delivers the lead forms you submit to us
  • Healthcare professionals, case managers or family members who contact us on your behalf
  • Our website server logs

Consumer health data we share, and with whom

We may share the categories of consumer health data listed above only as needed to provide what you asked for, or with your separate consent, with:

  • Service providers (processors) that store or deliver information for us: our email provider (Google), our website hosting provider, and Meta’s lead form tools.
  • Healthcare professionals, case managers or family members, but only those you ask us to coordinate with, and only with your permission.
  • Government agencies or others when required by law.

Sacred Life has no affiliates with whom we share consumer health data. We do not sell consumer health data.

Your rights

If you’re a Washington consumer, you have the right to:

  1. Confirm and access. Find out whether we collect, share or sell your consumer health data, get a copy, and get a list of all third parties and affiliates we’ve shared it with, along with an email address or online way to contact them.
  2. Withdraw consent. Withdraw your consent for us to collect and share your consumer health data.
  3. Delete. Have your consumer health data deleted. We’ll delete it from our records, including backups (backups may take up to six months), and notify the service providers and others we shared it with so they delete it too.

We won’t discriminate against you for using these rights. Responses are free, up to twice a year.

How to exercise your rights

  • Email: sacredlifehome@gmail.com, subject line “Consumer Health Data Request”
  • Phone: 360-721-4303
  • Mail: Sacred Life LLC, Attn: Privacy, 11412 NE 50th Ave, Vancouver, WA 98686

Tell us which right you’d like to use and how we can reach you. To protect your information, we’ll confirm your identity, usually by checking the email address or phone number you used to contact us. You don’t need to create an account. We’ll respond within 45 days. If we need more time (up to 45 more days), we’ll tell you why within the first 45 days.

Appeals

If we decline your request, in whole or in part, you can appeal by contacting us the same ways as above, with the subject “Consumer Health Data Appeal.” Please explain why you disagree. We’ll reply in writing within 45 days with what we did or didn’t do, and why. If we deny your appeal, you can file a complaint with the Washington State Attorney General’s Office: https://www.atg.wa.gov/file-complaint, or 1-800-551-4636.

Information covered by HIPAA

Protected health information that we collect, use or disclose under HIPAA (for example, residents’ medical records, if applicable) is not covered by this Consumer Health Data Privacy Policy. It is protected under HIPAA and described in our separate Notice of Privacy Practices.
